Educational Benefits Forms
All families enrolled in Minneapolis Public Schools need to complete the Educational Benefits Form.
Applications completed by our families...
- generate millions of dollars in funding for MPS and our schools, students and families.
- determine benefits for students and families like free bus cards and discounted internet service.
The income thresholds are higher than for some programs, so many families are surprised to find they DO qualify for benefits. If you wish to decline benefits, do so on the paper application, or email info.cws@mpls.k12.mn.us with their student's names.
Opt In to receive MPS messages
Transition Plus and MPS send messages pertaining to the whole school via text, email, and phone throughout the year to keep families informed.
Transition Plus families often find that their messaging has been turned off, so please take a few minutes to log into School Messenger to opt-in to messaging and to ensure your contact information is correct.
Options: log in or sign up in the upper right corner, download the app, or watch a tutorial under the app buttons
Talking Points App

Monday - Thursday: Classroom classes and worksites
Fridays: Community-Based Instruction (CBI).
Community Based Instruction (CBI)
Community Based Instruction is unique to Transition Plus! We firmly believe that by the time students get to us, they need to start transitioning to a community mindset. CBI helps with that by connecting with the community every Friday.
Monday through Thursday: students are learning skills needed to be successful adults in a classroom setting and/or at jobsites supported by Tplus staff.
Fridays: students join 5-week cohorts to dig deeper into a topic related to their IEP goals, classes, and interests. These cohorts meet on Friday mornings for 2.5 hours for 5 consecutive weeks to meet at community locations, engage with a community member at TPlus, or otherwise engage in a topic as a hands-on extension of their classes or as an IEP goal. We do not send home permission slips for community trips at Tplus because community trips are a consistent integral part of our programming.
When possible, students walk or take public transportation to access the community to practice these independent modes of transportation, but when these methods don't make sense, we take school buses.
Examples of Community Based Instruction include but are not limited to:
- Learn to ride/maintain a bike
- College or training program tours
- Employment searching
- Exploring parks that students can access outside of Tplus
- Connect with UNIFIED
- Budgeting
- Store navigation and shopping planning
- Living on your own - apartment maintenance
- Anime history
- Science lessons
- Video Game Design
- Relationships/boundaries
- Work-Based Learning
- Civics in the parks/Current events
- Garden to Table
Friday Afternoons:
Students are encouraged to practice social relationships by engaging in an interest-activity that they get to self-select with their peers. Students may lead the activity or engage with a staff-lead activity.
Some examples of Friday afternoon activities are:
- Chess Club
- Anime Club
- Esports Team
- Make-Up club
- Board games
- Karaoke
- Sewing/knitting
- outdoor walks/lawn games
- Student Voice (leadership group)
- More! - what will students come up with this year??
Transportation
Many students can learn to use the Metro Transit! Make steps to move toward independent transportation - either Metro Mobility or the city bus and biking. There are no school buses after Tplus!
GoTo (METRO TRANSIT) CARD:
Metro Transit cards (GoTo Card) are provided for our students with consistent attendance or who are attending classes at Minneapolis College.
- Questions can be directed to Daphne Breauxsaus via phone or email. 612-668-4079, Daphne.Robinson@mpls.k12.mn.us
- Many students can pick up their GoTo cards at Open House

Community Partners
Vocational Rehabilitation Services (VRS)
It is very important for all of our students to be connected with VRS while they are at Transition Plus. VRS helps people with disabilities connect to employment opportunities even after they graduate from Tplus and has many resources to help break down barriers to employment. Please support your student in connecting with VRS.
Southside Services
We are very excited to rekindle our partnership with Southside Services. They fill in the need gaps between school and social services. We'll have a Southside Services (SS) coordinator at Tplus a couple days per week to help with social service connections, metro transit training, soccer, Therapeutic Recreation, and more. Connect Now
Years ago, SS designed a young adult Therapeutic Recreation program for our students to connect with peers. Connect Now
Change Inc
Tplus is thrilled to have Change Inc therapists offering services at Transition Plus during the school day so our students have easy access to therapy services. If your student would like to access these services, please reach out to your Tplus case manager or reach out to Change Inc directly. https://www.thechangeinc.org/community-school-collaborative
MIGIZI
Transition Plus partners with MIGIZI to help our Native students connect to opportunities that help them discover their talents, creativity and dreams that benefit themselves, their families and community. MIGIZI is conveniently located just down the street from Tplus, so many students who work with MIGIZI can walk from Tplus.
