Required Annual Sports Physicals

LHS- March 3rd & 4th

AMJHS- March 5th & 6th

LHS West March 10th 8:00am - 9:00am

EBIS- March 10th 9:30am - 1:00pm for 6th graders only

Sports physicals will be given during P.E. classes. NO APPOINTMENTS ARE NECESSARY FOR THESE SPORTS PHYSICALS.

EVERY ATHLETE IN THE DISTRICT SHOULD BE RECIVING A SPORTS PHYSICAL ON THESE DATES, REGARDLESS OF PRIOR PHYSICAL STATUS. Receiving your yearly sports physical at this time will eliminate any need to make an appointment at UBMD, or come in on a Saturday before the Fall or Winter sports seasons to receive your sports physical.

In an effort to make this process more user-friendly for our families, Lockport Athletics has transitioned from SportsWare to FinalForms!

FinalForms must be completed by parent and student prior to March physical dates.

We have just launched this program, so please bear with us as we navigate any potential issues.

Helpful Notes:

• Parents must begin the registration process by creating their accounts, and inputting their child's information (parents of multiple athletes will be prompted to add additional children if necessary). Once the parent completes their registration and signs their portion of the forms, the student will then receive an email from FinalForms that their portion of the account is ready to be completed. (If the student uses a Lockport Schools email address for the account, please be sure to check ALL folders for the email. It may appear in your "Updates" folder instead of your inbox, etc.)
• The only sports season open for registration will be SPRING 2025. If your child is not a Spring 2025 athlete, please select "Coed Sports Physical" in the registration section. Registration for Fall 2025 sports will open in July, and registration for Winter 2025-26 sports will open in October.
• Parents of current 6th graders ONLY: Please register for the 2025-26 school year, and when it is time, please select "Co-Ed 2025 Sports Physicals" under sport registration.
• START DATE FOR FALL 2025 SPORTS: August 18, 2025
• START DATE FOR WINTER 2025-26 SPORTS: November 17, 2025
• 7/8th graders will not be able to select a JV/Varsity level sport without clearance from the Athletic Office. Please select the MODIFIED level of your sport until you pass the APP test. Once you pass the APP test, you will be manually added to the JV/Varsity level of your choosing by the Athletic Office. If you have existing valid APP test scores, please email Mrs. Gailor to add you to the JV/Varsity sport you would like.

With FinalForms, NO printed documents are required for physicals! Doctors will have access to your completed Medical History on your FinalForms account.

FinalForms must be completed by parent and student prior to March physical dates.

Please bring or wear your glasses to your sports physical.

Sports physicals will be valid for one year. This will include all sports for the Spring 2025, Fall 2025, and Winter 2025-26 seasons.

Any spring athletes who fail to get a sports physical on the above dates will have to make an appointment at UBMD Orthopedics and pay to have one done.
