Breast Cancer Awareness Month
Biometric Screening - Online Scheduling Instructions for UHC Enrolled Members
All registration support must be directed to the LGC customer support line:1-800-294-7151
UHC Enrolled members and their enrolled Spouses are welcome to register by following the directions below:
- Create an online account or login to www.myuhc.com
- In the blue tool bar at the top, click My Account > UHC Rewards
- Under the Activities section Click Get a Biometric Screening > Click Get more info
- On the next screen click Get Started
- Complete Let's Get Checked registration screens.
- Under On-Site Screening Event > Click Schedule Appointment
- On the next screen > Click Schedule your screening
- Consent to Participate page > Scroll down to click Accept
- Click confirm location.
- Click on the highlighted date in the calendar box to select and confirm an appointment time.
NOTE - Walk-In appointments will not be allowed. The online appointment registration will reopen the day of each event to allow members to schedule for any remaining timeslots.

Healthiest State Walk: October 2

October Wellness Challenge: Step/Activity Challenge
When was the last time you updated your Life Insurance & IPERS beneficiaries?
If you have had any recent life changes or have not completed a form within the last five years, it is important to keep your beneficiaries up-to-date. You can easily update your forms through Records.
- Log into Records: https://siouxcityschools.tedk12.com/sso/Account/Login?pid=2
- Click Available Forms.
- Click either IPERS Beneficiary Update Form and/or Life Insurance Beneficiary Update Form.
- Note both forms are for separate vendors and must be completed individually.

UHC Members - Vision Rider
UHC Members did you know that UHC has a vision rider that allows for one covered basic eye exam at the cost of your co-pay/deductible every two years. This service does not cover supplies such as frames, lenses, or contacts.

Hospital Indemnity - Need to File a Claim
If you elected the new Hospital Indemnity coverage during open enrollment or onboarding and need to file a claim you can choose any one of the options below to start the claims process. Those who also have medical coverage with UHC will be eligible for automatic benefit assist once an EOB has been issued. Those who are not enrolled will have to manually begin the process.
- Claims Portal - Log into www.myuhcfp.com to submit a claim
- Phone - Call 1-866-556-8298
- Hard Copy Email - Contact HR to receive a hard copy claim form and email to fpcustomersupport@uhc.com

Note: Payments will be issued via check and sent in the postal mail in a generic white envelope.
