This form is confidential. Only questions marked with a red asterisk * are required; other questions may be left unanswered, if desired. Information from this form goes into a confidential database within Clark County Public Health to document your participation in the training, and make you eligible for a naloxone kit from Clark County Public Health, if available.

In the two weeks following the training, we will get the La Center kits to Melissa Flatt and the La Center United Coalition for pick-up.
