The 5Essentials Parent Survey
Gathering input from our parent community is essential to our continuous improvements as a school community. We are contacting you today to share information about a unique opportunity to provide feedback through the Illinois 5Essentials Survey.
Please click here to complete the survey today for each school where you have a student enrolled.
Each of our schools and the District will use information gathered from the survey results to help identify areas of strength and areas for improvement. Please know the identity of respondents and their survey responses will be kept completely confidential.
The survey is not long and should only take about 10 minutes to complete. It will be open until March 31, 2023.
Thank you for taking the time to share with us your critical feedback.
Students in grades 4-8 also have the option of completing the survey. If you would like your child to participate, https://survey.5-essentials.org/Illinois/ and then click select “start here” under the student portion.
Students will need to log in using their student ID and their date of birth. Students who don't have a student ID yet or have other problems logging into the system can log in provisionally. Underneath the login page for students, click the link that says "Click here to take the provisional survey".
