Dear Parent(s)/Guardian,
As we prepare for the next school year, if your student will require emergency medications (rescue inhalers, epinephrine, diazepam, etc.) daily medications (insulin, Adderall, etc.), or as needed medications (sumatriptan, zofran, etc.) we kindly ask that you and your child’s physician complete the required medication forms that can be found on the Katy ISD website under Health Services. https://www.katyisd.org/domain/3795. These forms cannot be dated prior to July 1, 2025 and must have both parent and physician signatures. Only students with emergency medications and proper documentation may keep their medication with them. Outside of that, NO STUDENT IS ALLOWED TO HAVE MEDICATION OF ANY KIND IN THEIR POSSESSION. All medications must be signed in and kept in the clinic.
Also, if your child is entering the 7th grade and has not received the required vaccines, Tdap and Meningococcal, we ask that you complete that over the summer and send a copy of your child’s updated record to dayannenrodriguez@katyisd.org or kristilarcher@katyisd.org. All students must be in compliance in order to receive their class schedule and attend school.
