Consent Needed - Free Dental Sealants for Your Child
Dear Parents/Guardians,
We have a great opportunity for your child to get free dental sealants through our school, thanks to Pima County Health Department.
What are Dental Sealants?
Dental sealants are protective coatings put on the chewing surfaces of the back, adult teeth (molars) to prevent cavities. They are safe and very effective.
Benefits of Dental Sealants:
- Prevent Cavities: They protect teeth from cavity causing germs.
- Quick and Easy: The application only takes about 15 minutes.
- Effective: 80% of cavities can be prevented with sealants. Children without sealants have almost 3x as many cavities as those with sealants.
- Free: This program saves you money and helps keep your child’s teeth healthy.
- Time Saving: This service provided at school, minimizing the time out of the classroom and eliminate parent’s need to take time off work.
To get this free service for your child, we need your permission. Please fill out and sign the consent form by clicking the link below or scan the provided QR code with your smartphone’s camera:
Please fill out the form by 9/24/24 so we can schedule our visit to your child’s school. We will be returning to the school to provide this service to students once consent forms are received.
Don’t miss out on this great opportunity to protect your child’s teeth! If you have any questions, feel free to contact the Pima County Health Department Dental Sealant team at dental.sealant@pima.gov or call us at 520-724-7902.
Thank you for helping us keep your child’s teeth healthy!
