We are experiencing some difficulty with receiving payments without the order forms being completed. Please read the following carefully:
You must submit the Google Order Form to have your student on the list for pizza. Once you have submitted the form you should automatically receive a copy of your order form to your email as confirmation.
Square payments are not creating the order, they are a separate process.
Square payments must contain the student name so we can verify who the payment is for
The payment link will now be sent out via email only AFTER we have received your Google Order Form for pizza and put you on the pizza order list. Please allow time to complete this process…it may take us several days to process everyone's orders.
For October, we will give confirmations to your students' homeroom teacher to send home so you can be sure that we received your order or were able to match your payment with your student. If you thought you ordered and/or paid and do not get a confirmation from your student please email: FTA@mmautah.org and we will look into it.
November Pizza Google Order Forms must be submitted no later than October 25th to allow us time to process orders for November pizza.
