We are very excited to welcome you as a Shepard Spartan in the coming school year. Before your child walks into their building on August 19th, there are some health forms that need to be completed. Below you will find information regarding the current health requirements for entry into 6th grade.

Physical Examination - Due at the beginning of the 2024-2025 school year.

Immunizations - Due at the beginning of the 2024-2025 school year.

Dental Exam - Must be completed between November 15th, 2023, and May 15th, 2025

If you have more than one child, a form will need to be uploaded for each individual student. Please remember to upload all pages of the forms to meet the requirements.

Physical examinations are required for all students entering 6th grade. This physical exam must be recorded on the State of Illinois Certificate of Child Health Examination Form. Remember to check and sign the "Health History" portion for parent completion. It is not necessary to submit an additional SPORTS PHYSICAL form for Interscholastic Sports participation as this exam will meet the requirements for participation. It will expire 395 days from the date of the physical exam.

The State of Illinois requires students entering 6th grade to show proof of vaccination for Meningococcal, a dose of Tdap or Dtap after age 10, and a complete Hepatitis B series. Immunizations must be up-to-date, recorded on the State of Illinois Certificate of Child Health Examination Form, and administered as recommended by the Illinois Department of Public Health per State of Illinois mandates. Dental examinations are required for all students entering 6th grade. The examination must be performed by a licensed dentist, completed on the Dental Examination Record Form, and be on file in the school health office. To meet the 2024-2025 school year requirements, the dental examinations must be completed between November 15, 2023, and May 15, 2025.

Please note in lieu of the immunization and health examination, parents/guardians may submit the: Illinois Certificate of Religious Exemption to Required Immunizations and/or Examinations Form.
