SENIOR FINAL TRANSCRIPT REQUEST
The Class of 2024 Graduation Survey is MANDATORY for all seniors. Your BLUE senior clearance form will require a signature from a counselor.
The Senior Survey is your FINAL TRANSCRIPT request!
All students will receive an unofficial copy of their transcript when they pick up their diploma.
NOTE:
Final transcripts cannot be sent earlier than May 28.
HOPE transcripts will be submitted by June 30.
Attention Rising Senior Parents
The Posse Foundation will start accepting nominations on May 3rd for their 2024-25 scholarship program. Below I have provided you with a brief description of the program along with the selection criteria.
Posse Scholars receive four year, full-tuition, leadership-merit scholarships and go on to attend one of their partner colleges or universities. They go to these universities in groups of 10 students ("posses") and support each other through their four years on campus. This year, Posse Atlanta will recruit, select, and prepare 60 Scholars from Metro Atlanta to receive full tuition (not room & board), leadership scholarships to five schools: Bard College, Boston University, Brandeis University, Texas A&M University, The College of Wooster, and The George Washington University.
Selection Criteria:
A successful candidate for nomination will most likely have some of the following characteristics:
- Academic Commitment - Posse has no GPA/SAT/ACT requirement but does look for students who enjoy school and whose transcripts represent an upward trajectory
- Demonstrated Leadership - Posse views leadership in two ways:
- Traditional Leadership: Students who are on the e-board of clubs, athletes, dancers, members of the band, etc.
- Non –Traditional Leadership: Students who are unable to engage in traditional extracurricular activities because they have commitments at home (i.e. babysitting), in their community, or because they have a job.
- Fit for Posse – Students who want to go to college out of state. Students who want to engage in and contribute to a support network. Students who are interested in the schools.
Nominees should be Juniors who will be Seniors in the Fall of 2024.

Immunization Records NEEDED!
Attention: Parents of rising 11th grade students
The State of Georgia requires your student’s immunization records to updated. Please see the information below.
Per the Georgia Department of Public Health, effective July 1, 2021, children 16 years of age and older, who are entering the 11th grade (including new entrants), must have received one booster dose of the meningococcal conjugate vaccine (MCV4), unless their initial dose was administered on or after their 16th birthday.
To comply with state requirements, you must provide Campbell High School with an updated Immunization Certificate (Form #3231) by August 1, 2024. Students, who are not yet 16 by August 1, 2024, will have 30 calendar days from the student’s 16th birthday to provide the updated Certificate of Immunization. Students must meet this requirement, or provide acceptable exemption documentation, for them to be/remain enrolled in school. We cannot accept any other form and only a doctor licensed to practice in the State of Georgia may complete this form. You can obtain this form from your provider or the health department.
