5Essentials Survey
13% out of 20% needed: Why should parents take the survey?
The Illinois Report Card rating is a critical measure of our school’s overall performance and quality. It provides families, educators, and the community with valuable insights into how well our school is serving students. Here’s why it matters:
1️⃣ Drives School Improvement
The rating highlights our strengths and areas for growth, helping us set meaningful goals and implement strategies to improve student outcomes.
2️⃣ Influences School Funding & Resources
Schools with lower ratings may qualify for additional state and federal support, while high-performing schools demonstrate effectiveness in using resources efficiently.
3️⃣ Impacts Public Perception & Enrollment
Families use the Illinois Report Card to make decisions about where to send their children. A strong rating can attract new families and maintain community confidence in our school.
4️⃣ Measures Equity & Student Success
The rating reflects how well we serve all students, including academic growth, attendance, and school climate. It helps ensure that every student has access to a high-quality education.
5️⃣ Supports Accountability & Transparency
The Report Card ensures that schools are held to high standards and that families have access to clear, reliable data about their child’s learning environment.
By completing the 5Essentials Survey, parents contribute directly to this rating, influencing decisions that shape our school’s future. Your voice matters in making Conley the best place for students to learn and grow!

Student Placement Form
2025-2026 School Year
It is the time of year when the District 158 Elementary Schools begin to gather and organize information for classroom placements for the next school year. The following is the language in the student handbook that addresses student placement in District 158.
STUDENT PLACEMENT:
“Student placement is a function of building administration. Teachers work together with administration to place students in classrooms. Parents may provide written comments or information pertaining to their child’s learning needs by May 1st."
Please do not submit specific teacher names. We ask that learning characteristics of the child be the main focus of your request. Teachers do move grade levels, buildings, and districts so the characteristics are much more helpful. The final placement is at the discretion of building administration.
