You can complete online HERE from any computer, tablet or smartphone with an internet connection. (The zip code for John Bowne High School is 11367.)
Completing your form online is quick and easy. If you do not wish to file your application online, you can request a paper application from the school. Please note that it may take longer to process your application if you file via paper.
Families, regardless of income, are asked to complete a 2024-2025 Family Income Inquiry Form (formerly known as School Meals Form) so that our school can receive additional funding. You do not have to give any income information if you are certain that you don't qualify, however the NYC DOE asks that families submit the form. Income Eligibility Guidelines are HERE.
Forms submitted online are processed quickly and you will receive immediate confirmation that your form was submitted. Please retain the confirmation information.
If families qualify according to the information submitted on the Family Income Inquiry Form (formerly known as School Meals Form), their child will be entitled to:
- College Board and ACT fee waivers
- College application fee waivers
- A waiver of Advanced Placement (AP) exam fees (if they are waived)

Keeping your address and emergency contact information current with John Bowne High School is very important. We need to have accurate information for each child we serve; we rely on this information to contact you in emergencies.
- Home address: to change please come in (or send student) with 2 forms of proof from the last 90 days.
- Correct home, mobile phone, work number for guardians.
- At least one emergency contact. People who pick the student up during the day MUST be on the emergency contact list.
