Affordable Care Act 1095-C Employer Provided Form
1095-C forms were sent via postal mail on February 11, 2020. If you have not completed your taxes this year, you will want to take this form with you to your tax appointment. If you have already completed your taxes, you will want to place this form with the rest of your tax documents. This form does not get sent to the IRS, but does need to be with the rest of your 2019 tax documents.
The 1095-C forms are only given to those considered Full-Time employees, or working 30 or more hours a week.
The District’s insurance does meet the minimum value standard for insurance. If you have questions on your coverage, please contact Kim Smith (279-6692 Ext. 6121) or Kysa Hagemann (279-6692 Ext. 6120) in the insurance office.
Affordable Care Act 1095-B Insurance Provider Form
1095-B Insurance Provider forms have been suppressed by the IRS and will not be mailed out. UnitedHealthcare members may obtain the 1095-B on the www.myuhc.com member portal or by calling the number on their medical ID card.
**New Wellness Event - March 24, 25, & 26**

Preferred90 Pharmacy Program - Now Includes Walgreens
Effective immediately, the UnitedHealthcare Preferred90 Program will expand to include all Walgreens retail pharmacies - previously a member only had access to a 90-day supply of their maintenance medication via home delivery through OptumRx or a CVS retail pharmacy. The expansion of this network offering increases member access for 90-day maintenance medications to include home delivery through OptumRx, all CVS retail pharmacy locations and now Walgreens retail pharmacies.
As a reminder a member may receive a 30/31-day supply of their qualifying medication at any participating retail pharmacy, if they want to receive a 90-day supply of their qualifying maintenance medication they will have to utilize the home delivery option via OptumRx or a CVS/Walgreens retail pharmacy. If a member would like to know if their medication qualifies for a 90-day supply please encourage them to register/log in to their account at www.myuhc.com, the UnitedHealthcare App or call the customer service phone number located on the back of their ID card.
