BACK TO SCHOOL FORMS
ALL Forms are ONLINE
Log in to PowerSchool and verify your student's information for the upcoming school year. This process is required for all returning FHPS students. This is important for ensuring we have accurate and up-to-date information for all students for the 2024-2025 year, and it is critical to complete the transportation form if your student will need FHPS busing.
Please follow these essential steps:
1. Click here to log in to your PowerSchool account.
2. Select “Forms” on the left navigation menu.
3. Verify and update all of your student's information.
4. Verify residency.
5. Complete all required school forms.
6. If you have more than one student, you must complete and verify the information for each student.
If your student requires FHPS busing, the transportation form must be completed by August 4 to ensure transportation for the first week of school.
YOU MUST COMPLETE AND VERIFY ALL FORMS FOR THE FOLLOWING REASONS.
1. Transportation: If your student requires FHPS busing, you must complete this form by August 4 to ensure you are assigned a bus stop for the first week of school. If you complete the transportation form after August 4, you may not be assigned to one until after the first week of school.
2. Student Health: These forms ensure that buildings have accurate information in case of an emergency and that we can continue to provide a safe and secure learning environment.
3. Communication: Accurate information ensures you receive important communication from your child’s school and the district.
This will help ensure a smooth start to the school year.
If you have questions or need assistance logging into PowerSchool, please email parentportal@fhps.net or call FHPS at (616) 493-8550.
Thank you for your cooperation in this important matter.

Michigan lawmakers have approved the state’s fiscal year 2025 budget, which includes funding for universal free meals at school.
Although meals are FREE FOR ALL STUDENTS this school year, you are still encouraged to apply for free or reduced price meals.
This information is critical in determining the amount of money that our school receives from a variety of State and Federal Supplemental programs, including, but not limited to: Instructional Supports, non-Instructional services, professional learning for staff, parent and community engagement supplies and activities, and technology.
Your family may also receive benefits outside of school such as reduced fees for activities, discounted internet service, and more.
Please take the time to complete the form even if you think you will not qualify.
VOLUNTEER CONSENT FORM
Volunteers are an important part of our school community. many of our activities would not be possible without the involvement of family volunteers.
In our ongoing effort to keep our schools safe, FHPS requires volunteer parents and community members to apply for criminal background checks provided by the Michigan State Police through their Internet Criminal History Access Tool, or ICHAT.
All individuals volunteering in our schools must complete a form to have a background check and any response will be kept confidential.
YOU MUST COMPLETE A NEW VOLUNTEER CONSENT FORM EACH SCHOOL YEAR.
WE LOVE OUR BOOSTERS!
Ways to get involved as Parents of Northern High
FHN ACADEMIC BOOSTERS is made up of individuals with students attending Northern High School. Our focus is to promote and enhance academic success and achievement for all students at Northern. The group offers meetings that cover essential information on standardized testing, course selection, academic support, and the college search and application process. To learn more about our organization or to make a donation, click here.
FHN ATHLETIC BOOSTERS are volunteer individuals with an athlete participating at FHN - one from each team. We meet monthly and encourage participation at all levels from freshman, JV, and Varsity. We raise money to support 30+ FHN sports. Our three primary fundraisers are (1) Concession sales, (2) Special Events, and (3) Patron Donations. Each year we distribute $50,000 to FHN teams. The extra support for training and Player/coach development is very important to our coaches and athletes. To learn more about our organization or to make a donation, click here.
FHN BAND BOOSTERS is made up of individuals whose students are in the band programs at Northern Trails, Northern Hills Middle, and Northern High School. Our focus is to support the musical efforts of our students. Donations are used for the maintenance of school instruments, marching band camp, guest clinicians and concert performers, tuxedos, uniform accessories, as well as numerous scholarships for students to attend summer music camps. We strongly rely on the support of our band booster volunteers and booster donations. To learn more about our organization or to make a donation, click here.
FHN ORCHESTRA BOOSTERS is made up of individuals whose students are participating in Orchestra at Northern Trails, Northern Hills Middle, and Northern High School. We are a behind-the-scenes volunteer group that assists our orchestra directors and our students at their concerts and special events. In addition, we provide financial support for all our orchestra students through guest conductors, guest artists, supplemental music, dinners, field trips, and other special activities. We also provide scholarships for students to attend summer music camps. We rely enormously on YOU to support our orchestra program with volunteers and donations. Building our Future
The Forest Hills Public School district continues to achieve excellence — currently ranking as one of the best school systems in Michigan.
The vision of Forest Hills Public Schools is to create a new model of education which equips every student with the 21st century skills essential for success. If Forest Hills truly wants to create a new system of learning and maintain its leadership position in the decades to come, then it must attract research and development funding from people who want to invest in the future success of our students and schools.
Our goal is to attract investment funds to create opportunities for innovation. The Forest Hills Public Schools Foundation will provide the capital needed to fund the research and development of innovations that can lead to new levels of excellence. Once those innovations have been tested and proven, the Foundation can then provide the funds to scale them to the entire district.

The offered coverage is secondary to any student or family medical insurance.
In effect, the District’s medical insurance will cover medical costs for school-related injuries that are not covered by the family’s medical insurance as long as the injury falls within the range of coverage.
There is no longer a need for an athlete to purchase medical coverage. All athletes are covered under this program.
