The LSSC is partnering with the Houston County Health Department to offer free to low cost medical appointments, nurses are on-site to provide basic care. We can treat the following:

  • Mild/Moderate conditions
  • High Blood Pressure
  • Screenings
  • Diabetes
  • Vaccines
  • Physicals...and more!

SWD NOW COMP Waiver Application Assistance

Need assistance on NOW and COMP waivers? Join Laura Sugg of The Arc Macon, for this informative session. For more information contact Parent Mentor, Marina Jones at marina.jones@hcbe.net. This is a comprehensive walk-thru of the application and can be submitted onsite. Please bring the following documents:

  • Completed/signed application (don't worry if you don't know each answer - we can help!)
  • Psychological report that includes IQ score, adaptive skills testing (available from school).
  • Proof of citizenship (birth certificate, passport, or permanent resident card).
  • Copy of Social Security card
  • Copy of Medicaid and/or Medicare card
  • Copy of Social Security benefit information (if applicable)
  • Copy of guardianship documents (if applicable)
  • Copy of IEP (available from school).
