🏆Congrats Crivitz High School Trap Team
The Crivitz Trap Team took 18 Student Athletes to compete at the 2023 Wisconsin State High School Clay Target League State Tournament held at the Wisconsin Trapshooting Association Homegrounds in Rome, WI. Our goal going into the State Tournament was a Top 10 Finish and we did it (out of over 60 teams).
It being our second visit to State, we made a HUGE improvement from 44th place last year to 8th place this year.

Athletic Reminders/Updates for 2023-2024 Crivitz HS/MS Co-Curricular Activities
REQUIRED ANNUAL CODE MEETING –THERE WILL BE AN IN-PERSON CODE MEETING
FOR THIS SCHOOL YEAR. The date for the required in-person code meeting for the 2023-2024 school year will be MONDAY JULY 31 at Crivitz High School in the gymnasium starting at 6:30 p.m. If you are not able to attend in person, you will need to set up a meeting with the athletic director and/or coach to complete this requirement before the first day of practice of your respective season. There will be a limited number of dates available to make up this requirement. At least ONE parent/guardian must attend along with each student that wishes to participate in extracurricular activities in the Crivitz School District for the
2023-2024 academic year.
ACTIVITY/ATHLETIC PAPERWORK – This process will be done again through ONLINE registration. The direct link for the online registration site is https://crivitzhs-ar.rschooltoday.com/. Otherwise you can access the site through our district webpage under the athletic webpage www.crivitzwolverines.com. This will be made available on July 15, 2023. Please do not try to register before July 15, 2023.
Q: What do I need to do in advance, so that I am ready to go on that first day of practice?
You will need an alternate year card or a physical completed before the first day of practice. The PAPER form will need to be filled out and turned into the office. The alternate year card or physical card CANNOT be turned in online. The below forms will be accessible online (with the exception of the physical/alternate year card) under your family account.)
i) WIAA Athletic Permit Card (Physical Card) – This card certifies that the student has had a physical and is physically able to compete. This card is kept on file and is valid for two (2) school years for all Crivitz High School/Middle School athletic teams. The green card is the physical card; the yellow card is for alternate years. Schedule an appointment
with your doctor early in the summer! Not having a physical or having to wait for an appointment to open is normally the biggest problem for athletes not being able to start practice on time at the beginning of the season.
ii) CHS/CMS Activities Code Handbook (Parent and Student Activities Code Contract) – This contract obligates the student and the parent to comply with the rules as set forth in this Activities Code and the team standards as determined by the team coach. This contract is kept on file and is valid for one (1) year for all Crivitz High School/Middle School activities. (Complete at In-Person Meeting on July 31st , 2023)
iii) WIAA Athlete Eligibility Information Bulletin (AEIB) – this bulletin clarifies rules regarding age, academics, attendance, residence, transfers, physicals, training rules, amateur status, and activities outside of school. (Online Registration)
iv) Medical Information Disclosure (Release) Form – This form is required for the release of medical information to appropriate parties for the proper training, care and treatment of our student-athletes in addition to baseline concussion testing. (Online Registration)
v) Medical Information Form – This information is required before students may participate in any school activity. PLEASE FILL OUT ALL FORMS TO THEIR ENTIRITY. Physician’s names/address/contact number, etc. are all required. (Online Registration)
vi) Transportation Liability Form—Even if the student is riding the bus to and from away events, this form must be signed. PLEASE INCLUDE SPECIFIC NAMES OF INDIVIDUALS WHO COULD POTENTIALLY BE RESPONSIBLE FOR THE TRANSPORTATION OF YOUR CHILD THROUGHOUT THE SEASON. (Online Registration)
Q: Can I begin practice without having the required paperwork turned into the school office?
No! The WIAA requires a completed WIAA Athletic Permit Card (Physical Card) and online registration to be on file in our office for each athlete participating on an athletic team. In addition, our Board of Education requires that the signed code contract be on file as well as all the medical forms/fees in order to participate. ALL PAPERWORK IS REQUIRED TO BE
COMPLETED PRIOR TO PARTICIPATING IN ANY PRACTICE. If you are having issues with your online account, you may click on forgot password and a new password will be emailed to you.
