2020-2021 Student Athletic Physical Information
Parents/Guardians,
The UIL and Conroe ISD have reviewed and have approved the following paperwork (link provided) for next year regarding physicals for next year. For 2020-2021 school year only, if the student athlete has a completed physical from this year, they will only be required to fill out the Medical History and UIL forms, online. Incoming 7th graders, new students to CISD, and new participants for Athletics, Marching Band, Cheerleading, Drill Team, and ROTC will need to get a physical for 2020-2021. If you choose to use your child's 2019-2020 physical, please note that family history or other medical conditions may warrant the training staff to request further medical documentation and/or an updated physical.
We will be going to Online forms through Sportsware. Parents can also print out the physical if they need one. This program will help expedite the submission process and allow the Moorhead Coaches and the Caney Creek Athletic Trainers to communicate about the paperwork better.
Below is the Website for Sports Ware:
When completing the forms - the campus / school is Conroe ISD. (not Moorhead)
All of the information can also be found on the Moorhead Jr. High athletics page on our school website.
This link will guide you through the process of completing SportsWare registration and forms necessary to complete pre-participation physical.
Bring this form with you to your doctor's appointment.
Upon completion of the doctors visit please turn in your completed physical to the coaches during equipment pick-up or schedule pick up.
Again, EVERYONE must complete the 2020-2021 Pre-Participation Form and Medical History Form online!
If you choose to use your child's 2019-2020 physical, please note that family history or other medical conditions may warrant the training staff to request further medical documentation and/or an updated physical.
Moorhead families should direct any questions to Coach Pickett at bpickett@conroeisd.net or Coach Bartel at dbartel@conroeisd.net . Incoming 9th graders should email the Caney Creek athletic trainers at cfmiller@conroeisd.net .

It would be wise to set time limits, put rating levels in place, and limit App accessibility on your child's phone. These can be found under Settings / Screen time on Apple devices.
IOS/Apple users I recommend that you please take a few moments and go to:
Settings
then scroll down to USE SCREEN TIME PASSCODE. (it's in blue)
Set a passcode only you know.

Language Arts Summer Reading Project
In Conroe ISD, we encourage all students to read over the summer in order to enrich learning and provoke thought. Summer reading strengthens reading skills, increases academic achievement, fosters a love for reading, and empowers students to become life-long learners.
"What we know is that children who do not read in the summer lose two to three months of reading development while kids who do read tend to gain a month of reading proficiency..."

Moorhead Electronics Policy
Students are allowed to use electronics before school, during lunch, and after school. All devices, including phones, must be powered off and put away the rest of the day during the hours of 8:40am - 4:00pm. Vibrate is not off. Headphones and all other devices should also be put away.
Please do not call or text your child in school except during their lunch period which is fine and largely why students use phones during this time. We ask that you please contact the front desk and we will deliver the message to your child.
Please know that every room in the school has a phone that your child can use.
Students bring electronic devices to school at their own risk. The school is not responsible for the loss or theft of electronic devices, but we will do our best to help your child locate their device should it be lost or stolen.
