Only one form is needed per family. This information is confidential and is not shared with agencies unrelated to the school district. The funding form can be completed through the School Funding Jot Form or by completing a hard copy in English, Spanish, Russian, Arabic, Farsi, or Pashto and returing to the school office. Thank you!
