Survey for all 9th and 11th Graders
Dear PVPUSD Parent or Guardian,
Annually, the Palos Verdes Peninsula Unified School District administers the California Healthy Kids Survey (CHKS) to students in grades 5, 7, 9, and 11. This year, the CHKS survey will be given during your child’s English class one day between January 13th and January 17th. The survey includes questions about students’ background, alcohol, tobacco, and other drug issues, as well as questions about school safety and their learning environment. Student participation in the survey is voluntary. We hope that you will encourage your child to participate so that their knowledge and opinion can be used to improve our school.
All survey modules can be previewed in advance. Please visit the PVPUSD webpage to see what surveys are being offered this year and the link to view them: PVPUSD Healthy Kids Survey .
The survey is anonymous. No names or other identifying information is connected to the answers except for the school's name.
Parents and Guardians have the right to opt their child out of this survey. To opt your child out, please return the permission slip to your child’s English teacher no later than January 13th. More information about the survey and how to opt-out can be found here: CHKS 24-25 MS and HS Permission Slip
