***NEW RULE EFFECTING ALL STUDENTS GRADES 10-12***EACH YEAR prior to any practice or participation, a UIL requires a UIL PreParticipation Evaluation Medical History form signed by both student and a parent or guardian. You all had completed your physicals last year and only need a physical every 2 years.
***FOR INCOMING FRESHMEN 2020***
UIL requires a UIL PreParticipation Evaluation Medical History form signed by both student and a parent or guardian, and a Pre-Participation Evaluation Physical Examination (PPE) upon entering the 1st and 3rd years of high school. A physical examination signed by a physician, a physician assistant licensed by a state board of physician assistant examiners, a registered nurse recognized as an advanced practice nurse by the board of nurse examiners, or a doctor of chiropractic is required.
The PPE Medical History form (9-12) and PPE Physical Exam (9) must be completed prior to July 25. Forms to download & area to upload are below.
