Welcome to 4K!
We hope you are all staying healthy and are enjoying all summer in Wisconsin has to offer! We are looking forward to welcoming your 4K student(s) to one of our eight community sites on September 3, 2024. We are starting our thirteenth year of Four-Year-Old Kindergarten in the Waunakee Community School District, and we are very proud of our program. We partner with excellent sites and our teachers provide an enriching environment where your child will learn through play. The growth we see during the 4K year is remarkable, and we are eager to meet your child and start the year.
Birth Certificate
Certified Copy of your student's Birth Certificate: Please bring the Certified Copy of the Birth Certificate, along with the completed form (link below) to the Arboretum Elementary School, 1350 Arboretum Drive.
Any day during the summer from M-Th 7:00-3:00 (office is closed on Fridays)
Thursday, August, 1, 4:00-6:00 pm
We will not keep the Certificate; it will be viewed and returned to you. Please see links below for detailed information. Please DO NOT email or scan a copy of the Birth Certificate. We need to see the original in person.
Please fill out Sections 1-8 ONLY and bring with you. We will complete items 9 & 10 plus the signature the day you come in.
You will need a certified copy, not a photocopy. We will not keep it.
For information about ordering replacement birth certificates in Wisconsin, please see this website:https://www.dhs.wisconsin.gov/vitalrecords/record.htm
Immunization Records
Immunization Record: We will need immunization records for enrollment. You can provide these in one of two ways. You may either email a copy to mirandamoe@waunakee.k12.wi.us or you may bring a copy when you bring the birth certificate to Arboretum Elementary during one of our drop-in times listed above. Please see links below for more detailed information.
