Attn: 6th Grade Families
To the Parents/ Guardians of 6th Grade Students Attending Camp:
This is a reminder regarding medication requirements for camp. Either our school's medication form or your healthcare provider's form is acceptable.
If your child already has medication and a form on file with the school, these will automatically accompany your child to camp. Please ensure that any medications are packed in their original packaging with dosage information, along with the doctor-signed Medication Authorization Form. Place everything in a Ziploc bag labeled with the school name, teacher's name, and student’s name.
Submit all medications to the school health office NO LATER than September 27, 2024. Any medication turned in after this date WILL NOT be taken to camp with your child. Please see the attached letter for further details.
Kind regards,
