Homecoming Guest Form
The dance is for ISD 191 BHS, BAHS, and Virtual Academy students; grades 9-12, no middle school students. The BHS, BAHS, and Virtual Academy students may bring ONE guest from another school or non school as long as the guest is 20 years old or below. No age of 21 years old or older is allowed.
How to register a guest:
1. Give this form (https://tinyurl.com/Guest-Form ) to your proposed guest at least one week before the dance.
2. Proposed student guest must have his/her principal signature for approval. The guest’s Principal must fax/email this form back to BHS by (September 29, 2022; 3:00 PM). Fax: 952-707-2102; Return to BHS office at least two days prior to event. ATTN: RONNA JOHNSON; rejohnson@isd191.org
3. The guest must be able to show a student ID and/or driver’s license at the school event.
4. If your guest is a college student or presently working and not attending college, the Burnsville High School Host must have his/her parent/guardian’s signature and phone number, attesting to this permission for this guest to accompany the Burnsville High School student.
