Utah Providers Expiring June 30th, 2020

Any providers who expired March 31st or expire June 30th will need to submit their application for a state license by June 30th to avoid late fees and lapse in license. There will be no additional extensions given. This includes the state application and the NREMT certification applications.

If your license expired March 31st and did not get moved to June 30th you can still submit without a penalty until June 30th. Please contact our office so we can adjust the dates.

Utah Providers Expiring September 30th, 2020

If your license will expire September 30th, 2020 you will need to have your application for CME verification into NREMT by June 30th, 2020. Failure to do so will result in late fees.
