Does your student get headaches? Does your daughter have cramps every month? Are they an athlete with sore legs and arms?

Please feel free to send up a bottle of medication to keep in the clinic! Fill out the attached form and send in medication in the original container. Medication cannot be kept with the student in class unless it is an asthma inhaler or Epi Pen, and only with specific permission from their doctor.
