As we prepare to welcome groups of students on-site for learning and support, your partnership is critical to ensure we take appropriate measures to keep students, staff, and families healthy. We must work together to minimize the risk of transmission of COVID-19.

This packet is being shared with all hybrid in-person families and includes several items that will be critical to ensure the health and well-being of our students while transitioning back to in-person learning or coming on site as a remote student.

1. Self-Certification process: Please review this important information about our self-certification process at Churchill School.

2. Self-Certification symptom checker postcard: Post this reminder in a prominent location at home as a reminder to self-certify EVERY DAY.

On the first day of in-person learning, you will need to complete and send with your child the self-certification form that has been provided as part of this packet of information. All students will be issued a blue wristband which is to be worn every day, signaling that self-certification has taken place. Thank you for your support in keeping all of our students and staff healthy. We are in this together!
