We would appreciate your input regarding the balanced calendar, please take a moment and answer the Balanced Calendar Parent/Guardian Survey

The intent behind the survey is to provide all stakeholders with the opportunity to give input. Your feedback is very much appreciated and we will review and consider all feedback when making our final calendar decisions.

Please be mindful that there are many factors which impact the development of the calendar and that while we will consider all feedback, we may not be able to incorporate all suggestions and requests. Please be mindful that every option considered may impact other options that are listed.

All of the questions have multiple options, please feel free to check as many of the options that apply to you. Survey submissions are due by December 1, 2023 at 12pm.

-CMH will have open office hours at OAC. This is an opportunity for parents to come in on an as needed basis to make connections and answer any questions you may have about the services they offer.

-Reminder this is CMH of Ottawa County and they only service residents of Ottawa County. If your student resides in a different county and needs information about CMH, they can connect your family with the CMH in that county.

-Please email Brenda if you would like to schedule a specific time for your family to meet with the CMH staff on December 20, 2023. bseymour@oaisd.org

- This is a great opportunity to discuss transition planning. Remember - much like we base transition services around student postsecondary goals and unmet needs, CMHOC also bases the services they provide on eligibility, the goals an individual has for their life and the unmet needs after exploring all options available.
