Family Needs Assessment
The Social Workers of USD 462 are trying to assess the current financial and social emotional needs of the students and families that we serve in our district. We would appreciate it if you would take the time to complete this assessment to better guide us. As school social workers it is part of our job to help remove barriers that are prohibiting our students and their families from reaching their full potential, and to try to ensure all students have an equal opportunity to a happy and healthy learning environment. You may choose to remain anonymous during this assessment, but if any of these areas of needs apply to you or your family and you are in need of support, we do ask that you leave your name if you would like for us to reach out to you. These responses will go directly to the school social workers and will only be used to identify the common needs of our families and to establish systems that will be the most beneficial to our student and family's success. We appreciate you taking the time to assist us in these very important matters.
